How Much Hearing Loss do Patients Experience Immediately after Middle Ear Surgery? - Effects of Packing

Abstract
The objective of this study is to measure and analyze the amount and pattern of hearing loss resulting from packing in middle ear cavity (MEC) and external auditory canal (EAC) after middle ear surgery. We obtained pure tone thre-sholds up to 12 weeks after middle ear surgery in 15 patients with intact ossicular chain. Also in 15 volunteers, pure tone thresholds were obtained after packing only in EAC. Pure tone average (PTA) by air conduction (AC) increased significantly by a maximal value of 38.7 dB at the second postoperative day, by 35.0 dB at 1 week. PTA by bone conduction (BC) also increased maximally at the second postoperative day by 4.8 dB. The elevation of BC threshold was more pronounced at high frequencies. Packing of EAC without MEC packing resulted in elevation of AC threshold by 41.3 dB without change of BC threshold. For patients with intact ossicular chain, AC threshold increased significantly during the period of gelfoam packing after middle ear surgery. BC threshold also increased mainly at high frequencies.
